唾液变形链球菌感染水平与乳牙龋病现状及发展的相关性分析

Relationship between Streptococcus mutans in saliva and the existing status or development of primary dental caries

摘要: [摘要] 目的:观察唾液变形链球菌感染水平与乳牙龋病的现状及发展的相关性,评价Dentocult SM法作为龋活跃性检测方法的有效性。方法:以223名2~4岁幼儿为研究对象进行口腔检查,并采用Dentocult SM法检测唾液变形链球菌感染水平,计算患龋率、龋失补牙数(dmft)、龋失补牙面(dmfs)、龋蚀指数(CSI);两年后再次进行口腔检查,统计学分析相关指标。结果:两次检查的患龋率、dmft、dmfs和CSI均随Dentocult SM分度的增加而增加,dmft、dmfs和CSI两次检查的差值亦随Dentocult SM分度的增加而增加(P<0.001),而两次检查的患龋率差值在Dentocult SM各分度之间无统计学差异(P>0.05),Dentocult SM分度与两次检查的dmft、dmfs和CSI以及其增加值均呈高度正相关。结论:唾液变形链球菌感染水平与幼儿乳牙的患龋状况和龋病发展趋势之间存在显著正相关关系,Dentocult SM分度是一种可靠有效的龋活跃性检测方法。

Abstract: [Abstract] Objective: To study the relationship between the infection level of Streptococcus mutans in saliva and the existing status or development of primary dental caries, and evaluate the effect of Dentocult SM as a method of Caries Activity Test (CAT). Methods: The subjects, 223 children aged 2-4 years, were checked by oral examination and Dentocult SM test to detect the infection level of Streptococcus mutans in saliva. Caries incidence, decayed-missing-filled tooth (dmft), decayed-missing-filled surface (dmfs) and Caries Severity Index(CSI) were calculated respectively. One more oral examination was conducted two years later. And the relevant statistics was carried out. Results:The caries incidence, dmft, dmfs and CSI of the two oral examinations increased with rising Dentocult SM grade, and the difference of dmft, dmfs and CSI between the two oral examinations also increased with the rising Dentocult SM grade (P<0.001). There was no significant difference in caries incidence between the two oral examinations among different Dentocult SM grades (P>0.05). Dentocult SM grades were highly positively correlated with dmft, dmfs, CSI of the two oral examinations and also their increase in two years. Conclusions: There are significant positive correlations between the infection level of Streptococcus mutans in saliva and the existing status or development of primary dental caries, and Dentocult SM test is a reliable and effective CAT method.
